According to the police, Anthony Herrera, 21, of Wauconda, was driving a blue Ford van northbound on U.S. Highway 12 when he was hit head-on by a black Cadillac SUV that was traveling southbound in the wrong direction. The collision was so severe that both drivers had to be cut out of their vehicles by the firefighters. Herrera was taken to Advocate Condell Medical Center in Libertyville, where he later died from his injuries. The driver of the Cadillac, a 48-year-old man, was also hospitalized in stable condition. The police are still investigating the cause of the wrong-way crash and whether drugs or alcohol were involved.

This was not the first time that Herrera was involved in a car accident. Just six days before the fatal crash, Herrera was also involved in another crash on Interstate 10 in Thousand Palms, California. His car then caught fire and he was trapped inside. He was rescued by a passing motorist who pulled him out of the burning car and performed CPR until the paramedics arrived.
